By registering to attend this event in-person, I understand the hazards and risks of attending this event in person and the risks of COVD-19. I am familiar with all applicable rules and regulations regarding COVID-19.

?I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE (on behalf of myself and my business) the Forsyth County Chamber and its employees and our host and its employees from any liability related to COVID-19 which might occur as a result of my being on the premises and participating in the event.
